Students wishing to change personal information such as address and/or telephone number(s) may do so by one of the following methods.
For an address change, include the complete new address - Street/PO Box, City, State and Zip.
For telephone number change(s), include the complete new number - area code plus number.
All other personal information changes must be requested in person by visiting the Student Records Office in the Student Services building. A valid government issued photo ID must be presented when requesting this service. Students will be required to complete the Change of Personal Information Form.
Currently enrolled students who desire a name change will be required to provide legal documentation with proof of name change. Legal documents that we may accept for proof of name changes are legible certified copies that have a raised seal and certification by a United States government official. These documents include Marriage Certificate, Divorce Decree, Certificate of United States Naturalization, or United States Court Order approving the name change. For Social Security number changes, students must provide their original Social Security card and a valid government issued photo ID.
All Continuing Education personal information changes must be requested in person by visiting the Student Records Office in the Student Services building.
When making personal information changes through Student Records, the student's CVCCOneCard personal information is not automatically updated. In order to make changes to a CVCCOneCard account, BankMobile must be contacted directly by the student.
